Elderly Care

SRCF has a patient centric approach to supporting elders facing the dual challenge of old age and poor health – physical, mental, and social. Our Elder Care team (comprising a Doctor, Psychologist, Physiotherapist, Occupational Therapist, Nursing Supervisor, Nurses, and Home Care Medical Attendants) works in collaboration with the seniors and their families to address their long term care needs, with the aim of maintaining their functional independence, and in turn improving their quality of life. This includes:

  • Assessment of needs and formulation of care plan
  • Manage nutrition
  • Manage activities of daily living (bathing, grooming, and dressing)
  • Manage mobility
  • Encourage social and family interactions
  • Aid in memory
  • Accompanying patient to doctor/hospital
  • Disease and medication management
  • Manage incontinence
  • Manage Psychological problems
  • Caregiver education and coaching
  • Dental care
  • Managing safety
